River of Blood, codenamed Riverboat, is Back 4 Blood's third and final expansion of the Annual Pass that has been released on December 6, 2022.
It features an all-new campaign act, a new Mutation family dubbed Sentinel, one new Cleaner, one friendly Ally, eight exclusive character skins, twelve new weapon skins, new cards, weapons, accessories and more.
All playable content included in the River of Blood expansion is accessible by all players in a party as long as one player in that party has purchased the expansion.
On November 8, 2022, Turtle Rock Studios revealed the expansion's name, release date and an infographic to the public via Social Network Services.[1]
New Campaign Story: Act 6[]
Main article: Campaign
This expansion expands the Campaign with a sixth Act, which consists of one Chapter and five Missions.
This will expand the Campaign to a total of 6 Acts, 12 Chapters and 44 Missions.
Players who either own the expansion themselves or have at least one player on their team who has purchased this content can jump into this Act. Furthermore, players do not need to have completed any of the previous Acts to be able to enter Act 6. Only the restrictions mentioned above are in effect.
As the Ridden Hives in the Tunnels of Terror and Act 5 from the Children of the Worm expansions were already excluded from matchmaking via Quickplay, this rule is expected to also apply to Act 6, as this content is tied to the respective expansion.
Duffel Bags are also available in Act 6.

These Missions have also a chance to appear in the Mission Pool of the Trial of the Worm mode.
New Cleaner: Tala[]
Main article: Tala
Tala, a female Cleaner who is Derek's older sister and a former cultist member. She is available exclusively for owners of the third expansion. This Cleaner is not available in Swarm mode.
New Ally: Jeff[]
Main article: Jeff
Alongside Tala, a Ridden companion can assist the Cleaners with their battle against the Ridden. Jeff, a friendly Tallboy, is an A.I.-controlled ally who can be summoned via the newly introduced Whistle in order to wreck mayhem.
New Game Mode: Trial of the Worm[]

Alongside the expansion, a new cooperative PvE mode called Trial of the Worm became part of the free update slated for the same day as the expansion. In this mode players can choose a path along a chain of four Missions in order to earn Supply Points (
) along the way. Players will be able to add Corruption Modifiers or choose a more challenging route to increase the difficulty and boost the amount of Supply Points rewarded. The mode features a score system alongside an online Leaderboard.
New Threats[]
The expansion introduced Six new AI threats in the form of a brand new common Ridden variant the Armored Soldier and the Sentinel Mutation types which include the Lobber, the Sentinel, Swarmer, and Wailer.
In addition, another new Mutation will make its debut in the game in the form of the Harbinger.
Expanded Arsenal[]
New items and weapons have been implemented with this expansion and are available to any Cleaner squad.
The Cleaners' arsenal has been expanded by the following new items and weapons:
- Flamethrower: Expels a cone of fire incinerate Ridden that can be refueled by gas cans or propane tanks..
- LAW: A rocket launcher that deals a tremendous amount of damage.
- Jeff's Favorite Whistle: A dog whistle that will summon Jeff, a friendly Tallboy to assist the Cleaners in combat.
Also, new attachments that grant modifications to melee weapons and the Bow have been introduced.

Weapon Skins[]

In addition, owners of the expansion receive twelve different skins for their weapons in a bloodstained look along with the phrase "Whos next".
Like the exclusive outfits of the expansion, these skins are available to players immediately if they have purchased the Annual Pass, Ultimate Edition or the expansion individually.
