Mutations are the mutated variants of the Ridden. Through some unknown mechanism of the Devil Worm, a small percentage of the Ridden will undergo a dramatic mutation, eventually turning into one of these Mutations, each with their own unique inhuman abilities. They don't spawn as regularly as the Common Ridden do, but their abilities can easily wipe out a team of thr Cleaners if not dealt with quickly, making them the true teamwork testers of Back 4 Blood.

The Mutations have higher intelligence and more abilities than the Common Ridden. They will sometimes bide their time, waiting for Cleaners to reach ambush areas before attacking them. Several Mutations may also hide after attempting to attack a Cleaner.

Boss Mutations[]

Boss Mutations are distinguished by their extremely high health, which appears on the HUD, and high capacity for damage.

Breakers[]

Breakers are boss-type Mutation that possess incredible strength and deceptive agility.

Ogres[]

Ogres are boss-type Mutation of giant proportions and devastating strength.

Hags[]

Hags are boss-type Mutation that will devour any Cleaner that provokes them.

Abomination[]

The Abomination is a behemoth that will slam enemies to death with its massive tentacles or melt them down with acid.

Harbinger[]

The Harbinger is like the Abomination a giant Boss Mutation that uses ranged attacks to defend itself. Players will encounter it in the final Mission of the sixth Act within the River of Blood Expansion.

Tallboys[]

Tallboys are large-statured Mutations with an oversized, club-like arm. As their name suggests, they stand taller than other non-boss Mutations and are easily identifiable in a horde due to their height.

Bruisers[]

Bruisers are belligerent Mutations that can unleash a devastating combination of attacks.

Crushers[]

Crushers are aggressive Mutations that can capture isolated Cleaners in their grasp and squeeze them to death.

Rippers[]

Rippers are ranged Mutations that can hurl projectiles at Cleaners from medium range.

Stingers[]

Stingers are nimble Mutations that can spit acidic projectiles to encumber Cleaners. They are far more agile and mobile than other Mutations and are capable of leaping onto and climbing walls to reach their target.

Stalkers[]

Stalkers are nimble Mutations will grab hold of Cleaners and drag them away from their team. They lack the conventional projectile attack that other members of the family possess.

Hockers[]

Hockers hurls a particularly adhesive projectile that will immobilise any Cleaner that it hits.

Urchins[]

Urchins launch barbed mines at Cleaners which deal a small amount of damage when in proximity of a Cleaner.

Reekers[]

Reekers are heavily-bloated Mutations that can cover Cleaners in their entrails upon their death, summoning a horde of Ridden to attack. They have a heavier build than other Mutations, which also reflects on their inferior mobility.

Exploders[]

Exploders are unpredictable Mutations that can detonate themselves at any time.

Retches[]

Retches are nauseating Mutations that can spit acidic bile to create deadly hazards. Their bile will leave a temporary pool of acid in its wake that will deal continuous damage to Cleaners standing in it.

Shredder[]

Shredders are aggressive Mutations that will pull Cleaners next to them and start attacking. Upon death, they spew out acidic bile around them.

Sentinels[]

Sentinels are a family of Mutations that made their debut in the River of Blood expansion. They are heavily-mutated colonies of worms that behave as a single organism.

Lobbers[]

Lobbers hurl sticky bombs that damage and burn any Cleaners caught in its large blast radius.

Swarmers[]

Swarmers have the ability to spawn a swarm of incests to aid them in battle. They will take cover and begin spewing the infected insects to overwhelm opponents.

Wailers[]

Wailers births Gas Sacs that track any enemies foolish enough to get close.

The following Mutations don't belong to a dedicated family of Mutations.

Snitchers[]

Snitchers are Mutations that, when attacked, will unleash a horde of Ridden with their ear-splitting scream.

Sleepers[]

Sleepers are Mutations that are wall-mounted who will abruptly attack any Cleaner within range.

The Swarm[]

The Swarm are a horde of Ridden infected Insects that will eat away at anything that enters their territory.
