The Campaign is the core cooperative game mode in Back 4 Blood. The Campaign is broken up into different four acts, ten chapters and 33 missions.
With the upcoming Children of the Worm expansion the Campaign will be expanded the to a total of five acts, eleven chapters and 39 missions.
Players can either start from the beginning of an act or dive into a specific chapter or mission depending on their personal progression within the Campaign.
The Campaign can be played on four different difficulties: Recruit, Veteran, Nightmare and No Hope.
On Recruit difficulty, all missions can be accessed via checkpoints once they have been cleared at least once by the player.
The mission checkpoints on higher difficulties are far more stretched out, resulting into a greater challenge to clear an act in a playthrough alongside various other obstacles.
The mode can be played with a pre-made team, online matchmaking, solo via the Training Mode or alone by starting the game in Offline mode.
Playing the game online allows players to form teams from the following platforms:
- PC (Epic Games Store, Microsoft Store, Steam, Xbox PC Gamepass)
- PlayStation
- Xbox
If players want to play with another across platforms the game offers this activity via the "Crossplay enabled"-option.
Recruit

Recruit difficulty
This ain't no cakewalk, but everyone has to learn the ropes.
Cleaners have increased health, damage, and ammo capacity.
Reduced Ridden variery and lower difficulty level for events.

Friendly Fire: Disabled

Recruit is the lowest difficulty and is the base difficulty for Supply Points calculation. The difficulty is best suited for players who are either new to Back 4 Blood or zombie first-person horde shooters in general.
The challenges this difficulty poses can be overcome without any specific decks or organization.
Veteran

Veteran difficulty
You know what you're doing. Gear up and get out there.
Solid teamwork and custom decks will help you face the challenges ahead.
Cleaners have small bonues to health, damage, and ammo capacity.
Friendly Fire: Enabled
+75% Supply Points earned.

Veteran is the medium difficulty and offers more Supply Points than the Recruit difficulty. Veteran difficulty is best suited for players who are already familiar with the gameplay and decks of Back 4 Blood or zombie first-person horde shooters in general.
The challenges this difficulty poses can be overcome without any specific decks but requires organization and self-awareness of threats.
Nightmare

Nightmare difficulty
You might just be crazy enough to pull it off.
Cleaners take increased Trauma damage and have 1 less Life.
Ridden have increased health, damage, and greater variety.
Fewer additional resources when Continues are used.

Friendly Fire: Enabled and more dangerous
+150% Supply Points earned.

Nightmare offers even more Supply Points than the Recruit and Veteran difficulties.
Nightmare difficulty is best suited for players who no longer find any challenge on Veteran difficulty, have unlocked a high range of cards, have very good knowledge of mission and gameplay, and are competent individually and as a team.
This level of difficulty requires that each individual on the team is familiar with the mechanics and mission flow, and is able to form a synergy within the team using their decks to increase the chances of survival.
No Hope

No Hope difficulty
Bring custom decks, strong coordination and your best crew or don't plan to get anywhere in this difficulty.

— No Hope difficulty description
No Hope is the newest and currently the highest difficulty of the game.
It has been added as a free update alongside the Tunnels of Terror expansion DLC that got implemented on April 12th, 2022 with Patch 1.07.
This difficulty is aimed towards elite players to grant them a far more challenging experience.
Clearing the entire Campaign on this difficulty will reward players with the Grisly End banner.

Dr. Rogers' Neighborhood
Dr. Rogers' Neighborhood is the first chapter of Act 3. It consists of five missions:
Remnants
Remnants is the second and final chapter of Act 3 . It consists of four missions:
Act 4
The Abomination
The Abomination is the only chapter of Act 4 and the final mission of Back 4 Blood.
Act 5
Act 5 is a unreleased Act that will be introduced in the second Expansion Pack for Back 4 Blood.
It will consist of one chapter and six missions.
Other
The first Expansion Pack Tunnels of Terror while not a Act did introduce Ridden Hive Levels that can be played as alternate routes in chapter progression, the hive you enter is chosen at random with the exception of The Nursery which must be entered from another Ridden Hive
